Ever since Chapter 223 of the Jujutsu Kaisen manga, the final battle against the evil forces of Sukuna and Kenjaku began, and for almost 50 chapters, Jujutsu High has worked tirelessly to take down the King of Curses. Sukuna proved to be the most powerful sorcerer of all time as he defeated some of the strongest characters in the entire series in rapid succession, with only his first opponent, Gojo, proving to be a match for him.

However, despite this dominance, Sukuna suffered far too much damage in his battle against Gojo, so he was significantly weaker in his subsequent bouts. After defeating just about every other character in Jujutsu Kaisen, his fatigue caught up with him, leading to an eventual defeat in his fight against Yuji. The eventual battle between Sukuna and Gojo was foreshadowed at the very beginning of the series as a clash between the strongest sorcerer of all time and the strongest sorcerer of the modern era. Their battle certainly lived up to all the hype as both sorcerers showed why they were at a separate level from everyone else, showcasing unparalleled mastery of their Cursed Techniques and control over Jujutsu sorcery.

Eventually, Sukuna was able to figure out the only way to counter Infinity, which is how he killed Gojo at the end of this lengthy and amazing fight. However, he never got a chance to revel in his victory as the rest of Jujutsu High wasted no time in jumping into the fray to kill the King of Curses. Through the unending onslaught of the heroes and the fact that Sukuna never got the chance to properly recover from his fight against Gojo, he was finally defeated by Yuji toward the end, being separated from Megumi and disintegrating into the wind.

How Sukuna may have survived:

Sukuna's defeat at the hands of Yuji was an incredible moment to finally witness, especially after he seemed like the most unstoppable force of nature that no one could take down unless they were as powerful as Gojo. However, the King of Curses underestimated Yuji, who proved to be the most talented sorcerer that the series has introduced, due to him learning various advanced techniques in such a short time.

Moreover, Sukuna also underestimated Yuji in other regards. While he only ever saw him as an entitled brat who was an insult to Jujutsu sorcerers, Yuji is the only person who felt any sort of connection to Sukuna because of Sukuna's time in his body and their relationship as the only family members either of them have left. Because of this, Chapter 268 of the manga shows Yuji's kindness.

Sukuna... let's try this one more time. Living not by cursing someone, but existing alongside them.

Yuji picks up Sukuna as his curse form disintegrates into nothing and offers him the chance to continue on living within his body, just as it was before. Through these means, Sukuna would continue to live but would essentially be a prisoner within Yuji's body, unable to exist in the outside world to carry out his whims and hurt people like he used to do back in the Heian era.

While this does give Sukuna an out in terms of staving off death once more, it is highly unlikely that he would accept Yuji's offer. This is because Sukuna is a character whose entire purpose is to live according to his wishes, no matter what. His impassioned speech to Kashimo proved as much, in which he stated that to live life by one's own metrics and rules is the only way to do so and that he only lived how he did because that's what he truly wanted.

If he were still to exist within Yuji, it would act as a counter to everything that he stands for, since he would essentially be a prisoner who's unable to live life how he wants to. There has been no indication that Sukuna is outright afraid of dying, so he would much rather go out on his terms than carry on living in captivity.

Moreover, this still may have happened. As he was disintegrating away, he was never seen to have fully dissipated, instead it cut to a last-minute shot of Yuji's face after he gave Sukuna the option of living on if he wanted. While highly unlikely, there still exists some doubt about Sukuna's ultimate fate as he could have taken Yuji up on his offer. Apart from this, there's no other way for Sukuna to survive.
